Passport Night Information

Burke High School / Linard H. McCloud Fine Arts Building

Tuesday, January 13, 2026 @ 4pm - 8pm

To apply for a U.S. passport for a child under age 16, you must apply in person with the child and both parents/guardians. (See #5 below, if both parents can not attend)

 

Here is a summary of the steps outlined on the U.S. Department of State website:

Required Steps

  1. Fill Out Passport Application: Complete the Form DS-11, but do not sign it until instructed by an intake agent at the acceptance facility. (Forms also available on-site during Passport Night)

  2. Provide Evidence of Citizenship: You must submit an original or certified physical copy of the child’s U.S. citizenship evidence (long form birth certificate, naturalization certificate, etc.).

  3. Prove Parental Relationship: Submit documentation that lists the parents or legal guardians. (long form U.S. birth certificate, divorce or adoption decree, etc.).

  4. Present ID: Parents/guardians must present a valid photo ID (driver's license, passport, etc.).

  5. Show Parental Consent: Ideally, both parents appear in person. If one cannot attend, you must provide additional documentation like a notarized Form DS-3053.

  6. Provide a Photo: Submit one color photo of the child that meets specific size and quality requirements. (Walgreens and CVS offer quick passport photos)

  7. Calculate Fees: Determine the required application and execution fees. Note that these are often paid separately. (Fees listed below)

  8. Submit in Person: Passport Night @ Burke HS / Linard H. McCloud Fine Arts Center

 

Key Information

  • Passports for children under 16 are valid for 5 years.

  • You cannot renew a child's passport by mail; you must use Form DS-11 and apply in person every time.

Passport Fees for Children (Under Age 16)

Applying for a child's passport involves two separate payments. To ensure your application is processed without delays, use the tables and guidelines below to prepare your payment.

Note, the Passport Card is sufficient for our trip to Canada. ($15 / $35)

The application and facility should be paid at the same time, but separately. Money Order and Checks accepted. (No cash payments)
